The purchaser will make PAYMENT AT END OF ENGAGEMENT or earlier, MADE PAYABLE TO
DJCARL.com ENTERTAINMENT, LLC. There is a $25.00 charge for each day without payment
(including SAT & SUN) until the agent in FULL amount receives the payment. There will be an
additional fee of $30.00 for checks that do not clear with the bank. In the event of non-
payment of normal or late fees, the agent retains the right to attempt collection through the
court system. The purchaser will be held responsible for all court fees, legal fees and collection
costs incurred by the agent. *There will be a $10.00 service fee for each collection notice that
the agent sends to the purchaser due to non-payment.

The purchaser will provide the agent with a reasonably secure floor area with EASY ACCESS
(ramp and/or elevator access between the parking/service entrance and the set-up area).
If equipment must be (1) carried up stairs, (3) lifted onto a stage or (3) carried across a lawn,
then additional labor will cost $75.00 added to the negotiated total fee. The purchaser
is required to make sure there is one (1) clean square or rectangular shaped table set-up
two (2) hours before arrival to place of engagement to be located within twenty-five (25)
feet of two (2) 120-volt AC outlets (3-prong grounded with at least 80 amps available).
The purchaser will provide covering or protection from weather conditions such as direct
sunlight, rain or excessive winds.

In order to prevent liability resulting from an accidental injury to any individual attending
this performance, DJCarl.com Entertainment, LLC reserves the right to deny any guest access
to the computer, sound system, CDs, and supplies owned or leased by the agent. In the
event the disc jockey (DJ) believes an unsafe situation exists, he/she may call upon the
purchaser to remedy the situation. If no action is taken, the DJ may cease operation until
the situation is resolved. The purchaser is still responsible for payment in full.

If at anytime damage, theft or injury occurs to any equipment, music or personnel pertaining
to DJCarl.com Entertainment, LLC because of negligent acts of the purchaser, members, or its
guests; the purchaser, (your name) will be held
responsible to pay for the damages, lost or medical treatment. DJCarl.com Entertainment, LLC
will notify the purchaser immediately at the time of any occurrence.

I, (your name) do hereby agree to pay the remaining
fee for the disc jockey and entertainment services of DJCarl.com Entertainment, LLC. At no time
may I, (the purchaser) or DJCarl.com Entertainment, LLC, (the agent) cancel this legal agreement
unless due to acts of GOD, illness, or accidents ninety (90) business days before the
performance. Should the purchaser and agent both agree to cancel the performance
IN WRITING, the deposit is non-refundable. In some circumstances, the purchaser may be
required to pay in addition to the deposit any unrecoverable costs already incurred by the
agent. (But not more than the total fee agreed upon). A 60% buy-out fee will be required for
performances that are cancelled for no legitimate reason before the 90 business days period.
The DEPOSIT is $ . Each hour OVERTIME will be $300.00 (non-negotiable).
Overtime is due after the performance end time in cash or by check ONLY in addition to the
total payment balance. There is a ten (10) minutes grace period before overtime charges
are incurred.

The purchaser is responsible for paying any charges imposed by the venue. These charges may include, but are not limited to, parking, use of electric power or the fire marshal (for use of fog).

This agreement is not binding until signed by the purchaser and signed and received by the agent enclosed with the deposit. Any changes must be in writing and signed by both the purchaser and agent.

All cases of credit card fraud are automatically referred for criminal prosecution
